What is dandruff and how might it interact with hair loss

Dandruff is a common scalp condition marked by scaling, flaking and sometimes itch. It is often linked with Seborrhoeic Dermatitis, an inflammatory scalp condition associated with the yeast Malassezia and elevated sebaceous oil production. In medical terms hair loss, or alopecia, may occur for many reasons including genetic androgenetic hair loss, telogen effluvium (diffuse shedding), inflammatory scalp disease, and scarring processes.

A key study found a positive correlation between dandruff severity (measured by scaling index) and percentage of telogen resting phase hairs in men over five years. Another review noted that poor scalp health, including dandruff or seborrhoeic dermatitis, may result in increased early hair follicle cycling and reduction in anchoring of the hair shaft. On the other hand a systematic review concluded that for seborrhoeic dermatitis the evidence linking it to permanent hair loss was weak or inconsistent.

What this means for you if you have dandruff and hair thinning

The evidence suggests that while dandruff alone may not reliably cause permanent hair loss, it can contribute to increased shedding or thinning under certain conditions. Chronic inflammation on the scalp from persistent dandruff or seborrhoeic dermatitis may disrupt the normal hair cycle, increasing the proportion of hairs in the telogen shedding phase. Persistent scratching or damage to hair shafts and follicles in areas of inflammation can weaken the structural attachment of the hair, possibly accelerating shedding.

If you also have another cause of hair loss, such as androgenetic hair loss, nutritional deficiency, or a medical condition, the presence of dandruff may worsen the appearance of thinning. Importantly, if the underlying follicle remains viable hair regrowth is possible. If the follicle is damaged or destroyed, for example by scarring, then regrowth is unlikely.
